This Data Privacy Policy and its Addendum(s) describe how KolakerApps Inc. collects, uses, processes, and discloses your Personal Data through the use of Kolaker’s mobile applications.

From time to time, we may update our Policy to reflect required changes or updates. When we do so, we will post the changes or updates in our app or website for your information and reference.

If the changes or updates involve your Personal Data requiring your consent, we will obtain your consent 30 days prior to posting these changes or updates.

CONSENT

By signing up to be a KolakerApps Inc. customer and for as long as you are a customer, you consent to our collecting, processing, using, sharing, storing, and eventually destroying of your Personal Data for the purpose of providing you with the services you have subscribed to.

Should you be unable to supply us with the Personal Data requested, we may be unable to provide you with products and services you wish to subscribe to, and updates on Kolaker’s latest offerings.

• complete user profile or registration forms (such as your name, contact information, and other identification information where needed);
• provide business and professional information to assess your eligibility to provide services of our health and medical professionals;

• verify your identity through various means (such as social media logins, submission of selfie images, or independently verified payment card information); and,
• fill up demographic information in surveys (such as your age, gender, and other information you may volunteer such as your marital status, occupation, and income information).

As a customer of and until you terminate your subscription with KolakerApps Inc., you continue to consent to our using your Personal Data such as but not limited to as follows:

• Enable you to register with and subscribe to KolakerApps Inc.;
• Avail of the services of the Healthcare Professionals and Providers offered by the company;
• Provide you with other subscribed services and relevant services;
• Abide by any safety, security, public service, or legal requirements and processes;
• Send offers and other promotional communications;
• Notify you when Site updates are available;
• Send you information about additional healthcare services or general wellness from us or on behalf of our affiliates and trusted third-party partners;
• Improve the quality of healthcare through the performance of quality reviews and similar activities;
• to connect you with third-party providers of the Services;
• Fulfill any other purpose for which you provide us Personal Information; and
• For any other purpose for which you give us authorization.

Only Personal Data is necessary for the services that KolakerApps Inc. offers and which you have subscribed to will be obtained from you and processed to enable you to avail of the services of the Healthcare Professionals and Providers. Additional Personal Data may be required from you should you desire to avail of the other products and services offered, but only if with your consent and authorization.

Disclosure of Your Personal Data

By submitting the required Personal Information to us, you consent to collection, disclosure, and use thereof. You hereby expressly waive and release us from any and all liability, claims, causes of action, or damages arising from our legitimate use of the submitted Personal Information:
